Client Questionnaire

Tax questionnaire

The IRS requires us to ask these questions before we prepare your return. It takes about 5–10 minutes. Your answers go directly to your file with AMR Tax & Accounting.

Returning client?

If you filled out this questionnaire with us last year, we can load your answers. You'll just confirm what's the same and update what changed.

About you

Please do not enter Social Security numbers on this form.

What was your marital status on December 31? *

Did your marital status change during the year? (married, divorced, separated, or spouse passed away)

Which of these apply to you this year?

Select everything that applies. We'll only ask about the sections you choose.

Your income this year

Check every type of income you (and your spouse, if filing jointly) received. The IRS gets a copy of these forms, so please include everything.

Do you have the forms (W-2, 1099s, statements) for every income source above?

Children & dependents

Add each child or relative you want to claim.

Could anyone else (another parent, grandparent, relative) also claim any of these children?

Are you divorced, separated, or never married to the other parent of any child?

Is there a custody agreement, or a signed Form 8332 releasing the claim to the child?

Compared with last year, did custody or where any child lived change?

Which documents could you provide to show the children lived with you? (if the IRS asks)

Head of Household

Head of Household has strict IRS rules — please answer carefully.

If you were married on Dec 31: did your spouse live in your home at any time during the last 6 months of the year?

Did you pay more than half of the cost of keeping up your home this year? (rent/mortgage, utilities, repairs, property tax, insurance, food eaten at home)

Who lived in your home with you, and for how many months?

Did anyone else (family member, roommate, partner) help pay household expenses?

Self-employment / business

Questions we need to complete Schedule C accurately.

Do you have a business license, EIN, or other registration?

How were you paid? (select all)

How do you keep track of income and expenses?

Did you use a vehicle for business?

Do you have income not reported on a 1099 (cash, tips, etc.)?

College / education

Did the school issue a Form 1098-T?

Can you provide receipts for tuition, fees, books and required supplies?

Was the student enrolled at least half-time in a degree or credential program?

Had the student completed the first 4 years of college before this year?

How many prior years has the American Opportunity Credit been claimed for this student?

Childcare / dependent care

For the Child and Dependent Care Credit (Form 2441) we need each provider's name, address and tax ID.

Can you get each provider's address and EIN or SSN? (do not enter it here)

Was the care for a child under 13 (or a dependent or spouse who cannot care for themselves)?

Did you pay for care so you (and your spouse, if married) could work or look for work?

Did your employer pay for care or give you dependent care benefits? (W-2 box 10)

Prior years & records

Has the IRS ever reduced or denied your Earned Income Credit, Child Tax Credit, American Opportunity Credit, or Head of Household status?

Did you file Form 8862 afterwards to claim it again?

If the IRS asks, will you be able to provide documents that support your return?
